This page contains information on:


This page allows you or an approved representative to view your demographic details. All details are of the person the My Health Record is about and include:

  • First name;
  • Last name;
  • Individual Healthcare Identifier (IHI);
  • Date of birth;
  • Age;
  • Sex;
  • Address (this will not be seen by your healthcare providers);
  • Indigenous status;
  • Veteran and Australian Defence Force status; and
  • Contact number (this will not be seen by your healthcare providers).

If you wish to cancel your My Health Record, this is also done from the ‘Profile’ page.

 

How do I find the ‘Profile’ page?

You can find your profile by selecting ‘Profile’ on the ‘Profile and Settings’ page.
 

How can I update my profile?

The ‘Profile’ page displays the details of the My Health Record holder. You can update the Indigenous status, Veteran and Australian Defence Force (ADF) status and the contact number of the My Health Record holder on this screen.


To update any other information, please contact DHS on 132 011.

 

Indigenous status

To update the Indigenous status of the My Health Record holder, select the appropriate option and click ‘Save’.
You can choose from the following options:

  • No
  • Yes, Aboriginal
  • Yes, Torres Strait Islander
  • Yes, both Aboriginal and Torres Strait Islander

If you wish to clear the Indigenous status, click ‘Clear’ then ‘Save’.

 

Veteran and Australian Defence Force status

To update the Veteran and Australian Defence Force (ADF) status of the person the My Health Record is about, select the appropriate option and click ‘Save’.
You can choose from the following options:

  • No, I have never served in the ADF
  • Yes and I have a DVA file number
  • Yes but I do not have a DVA file number

If you wish to clear the Veteran and ADF status, click ‘Clear’ then ‘Save’.

 

Contact number

The System Operator will use this number to contact you if you provide one.

The contact number must be in the correct format as follows:

  • Australian mobile numbers – must start with 04 (e.g. 0400 000 000)
  • Australian landline numbers – must start with an area code (e.g. 02 6000 0000)
  • Overseas numbers – must contain a country code (e.g. +44 0000 000000)
  • Only contain numeric digits (0-9) and/or special characters + – ( ) (e.g. (02) 6000 0000)

To update the contact number, enter a number into the field provided and click ‘Save’.

Who can view my profile?

Your healthcare providers will be able to view your profile, with the exception of your address and contact number. All Nominated Representatives will be able to view your profile. Full Access Nominated Representatives and Authorised Representatives will be able to modify your Indigenous status, veteran and ADF status and contact number. You cannot restrict access to your profile. For more information on access controls see the ‘Privacy and Access’ help section.
